Overview
Sometimes, you may want to create a brand new form for Black Crow AI's Predictive Offer instead of editing the existing one. This could be for various reasons, such as:
- Offering a new incentive to users in exchange for their contact information.
- Modifying colors, styles, or language.
The below steps pertain to when you want to set a form as a permanent standard/default moving forward. If you want to make a temporary change we recommend scheduling a form, as described here.
Note: To be able to edit your Default Forms, they need to be rendered by either Klaviyo or Attentive platforms. If your Default Forms are rendered by another platform, follow the instructions here.

Changing the Default Form
Note: You are about to update your newly created Black Crow forms. When prompted to enter details about the forms, such as your form ID, ensure that this information pertains specifically to the Black Crow forms.
If you do not have a new Black Crow form, please follow the instructions below and choose the option that aligns with your current setup:
- Predictive Offer Creative Change - Klaviyo
- Predictive Offer Creative Change - Attentive
- Predictive Offer Creative Change - Attentive + Klaviyo
You can independently change the Default Form for each URL pattern.
For each URL pattern that you want to change the Default Form for:
- Navigate to Settings > Product Settings (or click here).
- Click Manage under "Predictive Offer (Welcome Flow).
- Click on the "Edit" button at the bottom right of the Default Forms card.

Provide the following details for the new form you want to set as the default:
- Name: A name for this set of forms. This can be whatever name you like. e.g. a description of what kind of incentive they provide.
- Black Crow Desktop Form ID: Enter the form ID for the desktop version from the platform you use to render forms (e.g., Klaviyo or Attentive). For Klaviyo, the ID should be alphanumeric and consist of 6 characters. For Attentive, the ID should be numeric and range between 6-7 characters. If you do not have separate forms for different devices, this can be the same as the Mobile Form ID.
- Black Crow Mobile Form ID: Enter the form ID for the mobile version from the platform you use to render forms (e.g., Klaviyo or Attentive). For Klaviyo, the ID should be alphanumeric and 6 characters long. For Attentive, it should be numeric and between 6-7 characters. If you don’t have separate forms for different devices, you can use the same value as the Desktop Form ID.
- Click "Save Changes to Default Form" once you provided the correct details. If you don't wish to move forward with the change, click "Cancel".
- If you see a warning message, it’s likely that the Form ID you entered is not valid. If you’re confident the Form ID is valid and you still see a warning message, please let support@blackcrow.ai know.
